so your first name is Alan and Last name is  Lincoln , right?         Your surname is Lincoln.

How old are you, date of birth, are you a resident, where do you live,

I need your history,: your past medical history,

Habits: do you smoke; how long had you been smoking; do you drink; how many units a week?

are you on any medication?

Any addiction?

Are your Immunizations up to date? Diphteria, Poliomyelitis, Tetanos, Measles, Mumps, Rubella, Shingles, Flu... Did you have your flu vaccination updated this winter?

Doc says:"After these general health questions I have to proceed to the clinical examination;

of the heart and blood pressure

of the abdomen

the neurological examination

the gynecological examination

-Ok doc...Top off? (Patients ask this short question sometimes, the same as they say I'm sick, (short for I fell nauseous, and suffering from my stomach (abdomen).

When the patient has undressed, the doctor proceeds with checking the blood pressure, listening to the heart, the lungs and the abdomen: this is the clinical examination/

The doctor notes down the findings of the clinical examination on his computer; the abdomen, the lungs, the cardiovascular system, the genitalia, the nervous system.

This is the first page of the health application;

Second page is Case history; if the patient has any desease currently active, and needing to be looked after.
