When the patient has undressed, the doctor proceeds with checking the blood pressure, listening to the heart, the lungs and the abdomen: this is the clinical examination/

 Musculoskelettal system


It is made of bones, muscles, senews or tendons, and nerves. (And arteries, veins, lymph,  etc)

You can have your bone broken, a broken bone is very painful, you have to fix the fracture rapidly.

When you sprain your joint, you don't have a broken bone: the problem, the swelling and the pain, comes from the tendons and ligaments you sprained. It's been over-stretched and now it needs repairing. To get back to normal. It takes a few days.

Not only accidents can affect the joints. Deseases can cause joint pains as well. From damage in the tendon or in the bone or both. There are joint infections which are called infectious arthritis.

You can need pain killers, or anti-inflammatory medications, or antibiotics..

-Maybe I sprained my ankle a little bit when climbing down the stairs yesterday but I could keep going. A few hours later I was in pain and some swelling developped on the side of my ankle, and I could not walk; I hope it is not broken; do you think I need X ray examination ?


-Lie down flat on your back on the couch please and relax and let me examine the joint: yes it is swollen and sensitive, tender, achy ; it looks looser when I try to manipulate it: ok it is a sprained ankle; you 'd better have an X ray. People your age, I'm sorry to say, have fragile bones; let's be careful.

LOEVA QUIZ 3 Respiratory system



1 People inhale so as to bring oxygen to the blood in the lungs, through the respiratory system which is made of the nostrils, sinuses, the throat, the breathing pipes (trachea, bronchial tubes or bronchos) to the pulmonary tissue; exhale so as to get rid of carbon dioxyde.

2 Smoking is a bad habit because it makes your finger turn yellowish and a good one because it help  smokers get rid of the mucus on their lungs and bronchos. 

3 Covid desease is an infectious desease that affected only smokers. It does not spread in non smoking communities: in retirement homes for example there were no covid infections.

Respiratory system:

a) Coughing

-Does the coughing come in bouts, is it a dry cough or a wet cough? Is it painful?

b) Sputum

-People with chronic bronchitis - and smokers of course- they bring up what is called "sputum".

People with tuberculosis would spit in sputum cups: it is disgusting.

Patients can cough blood sometimes because of their desease; they spit mucus with red or black streaks of blood

c) Dyspnea 

Those patients are breathless, they run short of breath. Dyspnea is what the doctors coll that symptom. Breathlessness.

People with asthma happen to wheeze; the wheezing is induced by exposure to wind and dust and pollens or after taking a medication or even the wheezing can be provoked by stress. It can be eased by some medicines ("ventoline" spray is Salbutamol; Steroid sprays can be needed on a daily basis to prevent the wheezing and ease the desease. The doctor decides according to the severity of the desease.

DEEP BEATHING AND RELAXATION

 

Relaxation techniques

Releasing tension

We all have parts of our body that carry tension - where do you have yours? Go through the list below and relax your body.

Forehead
Is it creased and frowning, or smooth and relaxed? Push your eyebrows up then let them fall into a relaxed position.

Jaw
Are your teeth tightly clenched? Let them come apart without opening your mouth. Feel your jaw muscle. Is it hard or relaxed? Let it go.

Tongue
Is it up against your front top teeth? Let it lie on the floor of your mouth.

Shoulders
Are they up round your ears, or relaxed? Push them up and then let them fall down relaxed.

Breathing
Is it rapid breathing from your chest, or slow deep breathing from your stomach?

Hands
Are they clenched like fists ready for a fight or floppy and relaxed?

Toes
Are they curled up tight or relaxed?

Deep breathing

Stress, illness and loss of muscle as we get older means that we often start to breathe with our chest. Chest breathing means we take short, shallow breaths that only fill the top part of our lungs.

We do this automatically when we are scared, in pain or exercising hard. When we are fully relaxed we breathe using our stomach or diaphragm. These breathes are longer and slower than chest breathing. They use our full lung capacity. This type of breathing helps our mind and body to relax. Sometimes chest breathing can become a habit.

Chest breathing for long periods can cause problems like:

  • palpitations (your heart rhythm is disturbed)
  • dizziness
  • chest pains
  • poor concentration and memory.

Learning to deep breathe again can help change this habit. Try this deep breathing exercise:

  • Lie or sit in a relaxed position and put one hand on your stomach, near your belly button and put your other hand on your upper chest. Relax your upper chest and shoulders. Now take a gentle breath in by gently pushing out your stomach. Try to keep your chest muscles and shoulders relaxed.
  • As you breathe out, try to relax. You can use your hand to press on your stomach to help a little. Try to keep your chest and shoulders relaxed.
  • Keep doing this for a couple of minutes. Stop if you get dizzy. Don’t take huge breaths. Try and breathe quite slowly.
  • If you find it difficult, you can make it happen by deliberately pushing out your stomach muscles when you breathe IN and relax your stomach as you slowly breathe OUT. Or, you can PUSH your stomach in with your hand as you breathe OUT.

Set aside five minutes, twice a day to practice this to move towards slower, smoother, stomach breathing all of the time.
